2023-2024 School Year - Forms required to participate in a sport - Physicals to be given after June 1, 2023
All of the required forms needed to participate in a sport are found in the Athletic Handbook. All forms must be completely filled out and signed to be eligible to participate. The Athletic Handbook includes the following forms which need to be submitted once per school year:

If a student is participating in a 2nd or 3rd season, the following form must be completed and turned in:
CIPPE Section 7 - Re-certification by parent/guardian form
If requesting an Activity Fee Waiver, the following form must be completed and submitted to the superintendent's office for approval:
